WORKSHOP REGISTRATION
For your convenience, we provide two methods of registration:
- Download the PDF form, fill it out and mail the form and your payment to address on the form
- You can register here (Credit cards accepted include VISA, Master Card, and American Express).
An additional cash discount is offered to attendees who pay by cash or check.
Since total participation is limited to approximately 150 registrants, you are encouraged to respond promptly.

Each registrant will receive a copy of the extended abstracts at the Workshop. This will be the only publication issued by the Workshop and no reference citations may be made to this publication.
Spouse/Guest Registration
Pre-registration forms are enclosed for your convenience. You can also register on-line by registering here.
Registration Fee:
$175.00 for each spouse or guest (12 years and over)
$50.00 for each child ages 6 to 11 years.
Children 5 years and under are free
Fee covers: Hosted Social Events (same as those listed above)
A separate spouse/guest program will be available in the mornings and evenings while the technical sessions are in progress. This Spouse/guest program may include additional fees depending on activity. These Spouse/Guest program fees are payable during registration on Sunday (December 9).
